In Agreement made with Accelerate Your Game,
LTD and between the above name Athlete ("Athlete") or the Athlete's Legal Guardian
if the Athlete is under 18 yrs. Old ("Guardian") (collectively, "Client") and Accelerate
Your Game, Ltd. ('Sports Trainer"), Client hereby retains Sports Trainer to assist
Athlete in improving his/her speed, conditioning, mechanics, and fundamentals. This
training will consist of programs such as proper instruction in warm-ups, stretching,
start techniques, first step explosion, acceleration, and overall endurance (collectively,
the "Training"). Client agrees to indemnify, hold harmless, defend, release and
forever discharge Sports Trainer, including its officers, successors, assigns, agents,
employees, and any person under its direction and control, from any and all liability
arising from or related to the Training, including without limitation damages of
any kind, cost, and attorney's fees. Client certifies that Athlete is in good physical
condition and can safely participate in the Training. Client acknowledges that all
physical training and activity, including the Training, involves the risk of injury.
Client hereby, and by Athlete's participation in the Training, accepts these risks
and further acknowledges that it is Client's obligation & sole responsibility to
notify Sports Trainer of any injuries and/or physical conditions that might limit
Athlete's ability to safely participate in the Training and to stop Athlete's participation
in any activity that may be injurious to Athlete. Notwithstanding the foregoing,
Sports Trainer may limit Athlete's participation in any activity at any time for
any reason.
